File a Petition

Special and late academic action often requires the approval of your College Dean's Office.

In general, retroactive actions and actions involving registration and filing are under the jurisdiction of the University's Registrars Office and you should consult them with questions about those processes.

Questions about course substitutions in your major should be directed to your major advisor.

You may download any of the following petitions and forms administered by the CBS Dean's Office they may be submitted in person or online. If you submit the forms via email please note the following:

  • Please scan and attach any required documentation pertaining to the form
  • We can receive online forms only from your UCD email address
  • An email from your UCD email address serves as an electronic signature
  • Forms and petitions will be processed within three to five business days upon receipt
  • You will be notified via email to your UCD address when the form or petition has been processed
